REQUEST FOR QUALIFICATIONS CITY OF MT. JULIET, TENNESSEE PIN 122004.00, Lebanon Road Sidewalks Design and Construction Engineering and Inspection Services The City of Mt. Juliet, Tennessee is issuing a Request for Qualifications (RFQ) from interested Professional Engineering firms in order to provide professional services for Design and Construction Engineering and Inspection (CEI) related to construction of sidewalks on both sides of Lebanon Road (SR-24, US-70) between Nonaville Road and Mt. Juliet Road (SR-171) in the City of Mt. Juliet. Services will be provided in accordance with the requirements of the Tennessee Department of Transportation's (TDOT) Multimodal Access funds and the Local Programs Development Office, including all applicable local, State, and Federal procedures and regulations. Any firm wishing to perform the work must be on TDOT's current list of prequalified consultants at the time of submittal and have completed the Local Programs' Level 2 CEI training course. A non-mandatory Pre-RFQ meeting will be held in the Public Works conference room at Mt. Juliet Public Works, located at 71 E. Hill Street, for all interested consultants at 10:00 am on Friday, September 18th, 2015. The meeting will be for the purpose of providing interested consultants with an overview of the proposed scope and to respond to consultant's questions. The submittals must be received on or before 2:00 pm, on Friday, July 31st, 2015. No late responses will be accepted. The City reserves the right to reject any and all responses. SCOPE OF SERVICES: Services shall include, but not be limited to, environmental documentation/coordination, ROW and Construction plans, ROW & utility certifications, environmental permitting, bid book and specifications, construction cost estimate, bidding phase services, conducting preconstruction and monthly progress meetings, full construction inspections in accordance with TDOT and FHWA regulations, material testing, processing of pay requests, change orders, DBE documentation, and project close-out documents. EVALUATION CRITERIA: The evaluation criteria to be considered for the Request for Qualifications are listed below. 1. Ability and relevant expertise of the firm's personnel to be used in performing the service. 2. Past experience in the required disciplines with other clients. 3. Qualification and availability of staff. 4. Demonstrated ability to meet schedules without compromising sound engineering practice. 5. Evaluations on prior projects, if available. 6. Size of project and limited or unlimited prequalification status. 7. Amount of work under contract with the City. 8. Whether the consultant can perform the work efficiently without compromising sound engineering practice. Interviews may be conducted at the discretion of the City. The firm, by submitting a response, certifies that to the best of its knowledge or belief, no elected or appointed official of the City of Mt. Juliet is financially interested, directly, or indirectly, in the purchase of services as described in this RFQ. The City of Mt.
